Since 1979, Community Hospice & Palliative Care has been the Compassionate Guide for advanced illness management, helping people live better with a chronic condition.

Individually and as a community, there is so much we can do to improve the quality of life for those with life-limiting illnesses. Each of us can experience comfort, well-being and meaning as we approach the end of life. And your Community Hospice can help.

In addition to providing hospice care, Community Hospice & Palliative Care offers several programs that help improve quality of life, such as Community PedsCare, a nationally recognized pediatric palliative and hospice program for children with life-threatening and life-limiting conditions; Community Hospice & Palliative Care Veterans Partnership, a program providing specialized care and support to address the unique needs of Veterans; Community Palliative Care, a consultative service for people with serious and complex illnesses that helps them make important health care decisions; and Community Care Choices, a CMS research model to help people with advanced illness find greater access to comfort care and support.

The Community Hospice & Palliative Care Neviaser Educational Institute provides education and other resources for health care professionals and the community about dealing with serious illnesses and their impact. The Community Hospice & Palliative Care Foundation generates philanthropic and community support for patient care services, family needs and community programs, supplemented with contributions raised by the Community Hospice & Palliative Care Thrift Shops (www.thriftshopjax.com).
